From the developer:
Brutal DOOM is a gore/gameplay mod for both Skulltag and GZDoom. This mod allows you to perform headshots. The Assault Rifle will only perform a minor headshot but the SSG or Minigun will completely blow up their heads. You can cut your enemies in half at the waist with the Chainsaw or decapitate them by cutting their necks to remove their heads. You can still gib Zombies and Imps but you can shoot a rocket near them and you will eliminate them.
You may want to check out more software, such as DooM Legacy, Lode Doomer or Doom Builder, which might be similar to Brutal Doom.
